Roots & Routes is Fyrebird Recovery’s harm reduction case management program, offering flexible levels of support that meet people where they are and move at the pace they choose.
Support is not one-size-fits-all. People may need different kinds of care at different times — from immediate crisis support to longer-term navigation. Movement between levels is based on need and choice, not compliance or readiness.

Immediate, low-barrier support for urgent moments
Level 1 is designed for times when someone needs help right now. This level focuses on immediate safety, stabilization, and problem-solving without long-term commitment or intake barriers.

Level 2 supports individuals who want help addressing specific barriers that are impacting daily life. This level focuses on reducing instability and increasing access to care and resources.

Level 3 is for individuals navigating multiple, overlapping challenges who want consistent support over time. This level centers trust, advocacy, and continuity of care.

The Second Chance is a six-month, structured intensive case management program for individuals seeking long-term stability and permanent housing.
This level provides comprehensive, wraparound support and a consistent structure designed to support growth, skill-building, and long-term planning.

Program contribution: Program costs are discussed collaboratively as part of individualized financial planning with case management. Contributions are adjusted based on income, resources, and ability to pay.
No one is denied consideration solely due to financial barriers.
