what is the routes and routes Program?

Roots & Routes is Fyrebird Recovery’s harm reduction case management program, offering flexible levels of support that meet people where they are and move at the pace they choose.

Support is not one-size-fits-all. People may need different kinds of care at different times — from immediate crisis support to longer-term navigation. Movement between levels is based on need and choice, not compliance or readiness.

ROOTS AND ROUTES PROGRAM LEVELS

🔥 Level 1: Roots — Crisis Support

 

Immediate, low-barrier support for urgent moments

Level 1 is designed for times when someone needs help right now. This level focuses on immediate safety, stabilization, and problem-solving without long-term commitment or intake barriers.

Support may include:

  • Emergency transportation
  • Immediate safety planning
  • Short-term crisis navigation
  • Rapid referrals and warm handoffs
  • Emotional support and grounding

What to expect:

  • No lengthy intake or documentation
  • Support may be one-time or repeated as needed
  • No requirement to continue services

This level exists to help people get through the moment safely.



🌱 Level 2: Roots — Stabilization Support

Short-term navigation and goal-focused support

Level 2 supports individuals who want help addressing specific barriers that are impacting daily life. This level focuses on reducing instability and increasing access to care and resources.

Support may include:

  • Appointment coordination and reminders
  • Assistance with documentation and applications
  • Healthcare, housing, and benefits navigation
  • Harm reduction planning
  • Follow-up and check-ins

What to expect:

  • Time-limited support based on participant goals
  • Flexible communication and scheduling
  • Goals are participant-led
  • No requirement for sobriety or behavior change

This level creates breathing room and helps reduce ongoing stressors.

🧭 Level 3: Routes — Intensive Navigation

Level 4: Routes — The Second Chance Intensive Case Management Program (6 Months)

Ongoing, relationship-based case management

Level 3 is for individuals navigating multiple, overlapping challenges who want consistent support over time. This level centers trust, advocacy, and continuity of care.

Support may include:

  • Long-term care coordination
  • Systems navigation and advocacy
  • Accompaniment to appointments when appropriate
  • Support during transitions (housing, treatment, reentry, health changes)
  • Regular check-ins guided by participant preference

What to expect:

  • Relationship-based support
  • Flexible timelines
  • Trauma-informed pacing
  • Movement between levels as needed

This level honors progress that is non-linear and individualized.

Level 4: Routes — The Second Chance Intensive Case Management Program (6 Months)

The Second Chance is a six-month, structured intensive case management program for individuals seeking long-term stability and permanent housing.

This level provides comprehensive, wraparound support and a consistent structure designed to support growth, skill-building, and long-term planning.

Program components include:

  • Housing referrals and housing planning
  • Financial planning and budgeting support
  • Career development and employment readiness
  • Mental health and medical care planning
  • Evidence-based interventions
  • Structured schedules
  • Volunteering and group engagement

Basic needs provided throughout the program:

  • Hygiene supplies
  • Food and meals
  • Bedding and essential items
  • Transportation
  • Housing support

Program outcomes focus on:

  • Pathways to permanent housing
  • Financial literacy skills
  • Career goals and next steps
  • Ongoing healthcare and support plans

APPLICATION PROCESS LEVEL 1-3

 Levels 1–3 of Roots & Routes are designed to be accessible and low-barrier. Support is based on need, availability, and participant choice.

Who this is for:

  • Individuals seeking crisis support, stabilization, or ongoing navigation
  • People who want support without long-term commitment

What to know:

  • No program fees for Levels 1–3
  • No sobriety or treatment requirements
  • Services are voluntary and nonjudgmental
  • Participation may begin immediately or through referral

How to access:Contact Fyrebird Recovery directly or connect with staff or peer navigators during outreach to discuss current needs and next steps.

APPLICATION PROCESS LEVEL 4

 

The Second Chance Initiative Program

The Second Chance is offered through an application and approval process to ensure the program is the right fit for both the participant and the level of support provided.

Eligibility notes:

  • Currently available to men only
  • Program length: six months
  • Enrollment is limited

Program contribution: Program costs are discussed collaboratively as part of individualized financial planning with case management. Contributions are adjusted based on income, resources, and ability to pay.

  • Initial program contribution: $220 
  • Additional costs, if any, are discussed transparently and planned for together

No one is denied consideration solely due to financial barriers.
